AB  - Jede vierte erwachsene Person in Deutschland ist von Adipositas betroffen. Dabei steigt diePrävalenz dieser chronischen Erkrankung weiter an. Mit den in Kraft getretenen Richtlinien fürein Disease Management Programm (DMP) Adipositas steht der Behandlung der Erkrankungeine neue Option innerhalb der deutschen Versorgungslandschaft zur Verfügung, die einenmultimodalen Ansatz verfolgt und Betroffene beim Selbstmanagement der Adipositas unter-stützt. Die Inhalte der Richtlinien zielen darauf ab, das Körpergewicht zu reduzieren bzw. zustabilisieren und die transsektorale Zusammenarbeit hinsichtlich Adipositas-assoziierter Komor-biditäten zu verbessern sowie letztlich Morbidität und Mortalität zu reduzieren.

AB  - Type 1 diabetes can be identified years before clinical diagnosis based on the detection of multiple islet autoantibodies. Based on this and depending on glycaemic status, a distinction is made between the pre-symptomatic stages 1 and 2. In scientific studies, early detection was associated with significant health benefits at the time of diagnosis and beyond. Teplizumab, an anti-CD3 monoclonal antibody, is the first drug available in the US that has been shown to delay the onset of diabetes in stage 2 by more than 2.5 years. It may soon be available in Europe. This perspective is fuelling the debate about extending population-based screening, but it also requires ethical considerations and a clear public health framework for the medical and psychological care of those affected.

AB  - Type 1 diabetes incidence has been increasing every year, especially in infants; the disease is often diagnosed at an advanced stage with diabetic ketoacidosis already present. Diagnosis at an early stage can prevent complications, and is one of the objectives in the Fr1da study, the first project of its kind worldwide that has screened more than 33000 infants aged two to five years for islet antibodies since January 2015. The first three cases that were diagnosed with an early stage of type 1 diabetes (positive for two or more islet autoantibodies) have already been included in a structured follow-up programme. The present contribution will cover these cases as examples for the process from positive screening result to individual prediabetes training.
