Influence of perinatal factors on the appearance of islet autoantibodies in offspring of parents with type 1 diabetes.

Evidence for a very early initiation of islet autoimmunity in relatives at risk for type 1 diabetes (1) implies that putative risk factors associated with type 1 diabetes operate within the first years of life. Recent investigations have therefore focused on this perinatal period (2–8). Here, the association of several perinatal factors with islet autoantibody development was examined in 1043 offspring of parents with type 1 diabetes (671 from mothers, 341 from fathers, and 31 from both parents with diabetes) prospectively followed from birth until at least 2 yr and up to 11 yr of age (BABYDIAB Study, median follow-up 3.5 yr) (1). Persistent islet autoantibodies [insulin autoantibodies (IAA), glutamic acid decarboxylase antibodies (GADA), tyrosinphosphatase IA2 antibodies (IA2A), and/or islet cell antibodies (ICA)] have developed in 47 (4.5%) of these offspring by 2 yr of age and 12 offspring (1.2%) developed type 1 diabetes (1).
