The incidence and prevalence rates of metabolic dysfunction-associated steatotic liver disease (MASLD) are highest in countries with the highest rates of obesity and type 2 diabetes. In Germany, the combined prevalence of all steatotic liver diseases is around 21% in women and 36% in men, MASLD being present in 90% of these cases. An increase in MASLD of 4.5%–5% by 2040 is assumed for Germany, whereas an increase in advanced metabolic dysfunction-associated steatohepatitis (MASH) up to cirrhosis and liver transplant of at least 20% is predicted—with corresponding effects and costs for society. Early detection of MASLD in high-risk groups, e.g., obesity, dysglycemia (prediabetes), and manifest type 2 diabetes, is essential. Diagnosis of MASH with grade 2 or 3 fibrosis is of particular relevance, as drugs have now been approved for this indication. Significant hurdles to improvements in diagnosis include the perception of MASLD as well as its hepatic and extrahepatic consequences by the general population but also by medical professionals and policymakers, the availability of resources, defined care pathways, and transparent reimbursement of diagnostic tests. Prevention of MASLD is only possible with simultaneous prevention and treatment of obesity, prediabetes, type 2 diabetes, dyslipidemia, and arterial hypertension, which underscores the dominant role of lifestyle changes and weight reduction. As the effects hereof are often insufficient or temporary and because the pathogenesis of MASLD is highly heterogenous, which is also relevant for its consequences, drug combinations are increasingly considered for MASLD/MASH treatment. Successful management of MASLD and its complications is only possible with close interdisciplinary collaboration. Diagnostic tests must be newly and further developed, digital systems integrated, and local regulations observed; consideration of societal and individual characteristics with the aim of achieving equality is essential, as are research, education, and training.
